How to Request an Independent Assessment from Western Medical Assessments.
1. Download the Forms by clicking HERE. You will need Adobe Acrobat Reader to view the PDF files. IF you do not have the software click the icon below to download it.
2. Fill in the Western Medical Request Form (we will personalize one for you).
3. Attach medical documentation and fax directly to us at 780.433.1127. If the number of pages exceeds 25 sheets, please courier to:

4. When information is faxed, there is no need to send original copies through the mail.
5. If you have not heard from Western Medical within 48 hours, please call us directly at 780.433.1191 or at 800.290.2189 to confirm that we did receive your documentation.
